TCF and IELTS Academic both test Listening, Reading, Writing and Speaking. The difference is context: TCF uses healthcare situations and profession-specific Writing and Speaking tasks, while IELTS Academic uses general academic topics. If your regulator accepts both, choose the one that fits how you use English at work. For most practising clinicians, that is TCF.

When IELTS may suit you better

- Check acceptance. Open your regulator's list of approved English tests and note the score each one requires.
- Try a sample of each. Take a timed TCF sample test and a timed IELTS practice test, and compare how comfortable each format feels.
- Pick the one with the smaller gap. The better test for you is the one where your practice results are closest to the required score.
